Home Health & Hospice Physical Therapist

VitalCaring is seeking a Physical Therapist to deliver compassionate, evidence-based care to home health and hospice patients. In this field-based role, you will assess functional status, design and implement individualized treatment plans, and help patients regain mobility, manage pain, and safely remain at home. You’ll collaborate closely with an interdisciplinary team, educate patients and caregivers, and document clinical progress.
