Community Health Registered Nurse

Centerpoint Health in Middletown, OH is seeking a Registered Nurse to deliver compassionate, patient-centered care in a community health setting. The RN will provide primary and behavioral health nursing support across the lifespan, including assessments, triage, vaccinations, care coordination, and chronic disease management. Working within an integrated care team, you’ll collaborate with providers, behavioral health, and dental staff to support underserved populations. Responsibilities include patient education, documentation in the EHR, quality and safety initiatives, and participation in care planning in a mission-driven, collaborative environment.
