Physical Therapist, Home Care

VNS Health seeks a dedicated Physical Therapist for home care in the Bronx. In this role, you’ll evaluate patients in their homes, develop individualized treatment plans, and deliver evidence-based interventions to restore function, mobility, and independence. Responsibilities include gait and balance training, therapeutic exercise, fall prevention, and pain management, as well as educating patients and caregivers on safety and home exercise programs. You’ll collaborate closely with an interdisciplinary team, document care in EHR systems, and uphold high standards of compassionate, patient-centered service in the community.
