The LPN works under the direction of a Registered Nurse to deliver patient-centered care, support treatment plans, monitor patient conditions, and assist with admissions, discharges, and patient education.

Position Details:

Key Responsibilities

  • Provide direct patient care within the LPN scope of practice.

  • Assist with patient admissions, discharges, assessments, and care planning.

  • Monitor patient conditions and promptly communicate changes to the RN.

  • Participate in bedside shift reports, patient rounding, and interdisciplinary collaboration.

  • Reinforce patient and family education.

  • Delegate and support patient care activities for unlicensed assistive personnel as appropriate.

  • Maintain accurate documentation and follow hospital policies and procedures.

  • Complete required competencies, certifications, and ongoing education.

  • Contribute to quality improvement initiatives and department goals.

Qualifications

  • Graduate of an accredited Practical Nursing program.

  • Current LPN license in the state of practice.

  • New graduates and experienced LPNs encouraged to apply.

  • Strong communication, teamwork, and patient care skills.

Certifications

  • BLS (American Heart Association) required within 30 days of hire.

  • IV Certification required upon hire or within 3 months of hire, based on department requirements.

  • Additional certifications (ACLS, PALS, PEARS, S.T.A.B.L.E., or other unit-specific certifications) may be required based on assignment.
