Outpatient Primary Care Nurse Practitioner or Physician Assistant

Community Care Physicians, a physician-owned multispecialty group in Latham, NY, seeks an Outpatient Nurse Practitioner or Physician Assistant to provide patient-centered care in our Capital Region practices. In this role, you’ll manage acute and chronic conditions, perform comprehensive assessments, and coordinate care across primary and specialty services.
