POSITION SUMMARY
The Nursing Supervisor & Educator is responsible for the provision of coordinated quality patient care and the development, coordination, and delivery of educational programs. This role combines responsibilities in orientation, training, supervision, and clinical performance evaluation of home health staff. The position also involves direct oversight of Home Health Aides (HHAs), Personal Care Aides (PCAs), and nursing staff both in classroom settings and field environments.
RESPONSIBILITIES
- Carries out the agency’s mission, philosophy, goals and objectives within guidelines of agency policy and position function.
- Educational & Training Duties:
- Develops, coordinates, facilitates, and implements orientation and ongoing education programs.
- Designs and implements curriculum for HHAs, PCAs, and nursing staff in compliance with NY State Department of Health standards.
- Provides classroom and practical instruction.
- Prepares testing materials to evaluate student performance.
- Supervises skill performance for PCAs/HHAs.
- Maintains accurate training and student records.
- Conducts in-service training in accordance with DOH guidelines.
- Participates in performance evaluations, goal setting, and improvement planning.
- Engages in performance improvement initiatives as needed.
- Supervisory & Clinical Duties:
- Conducts field visits to clients' homes to assess/reassess, supervise the Aides and provide training support, and in order to complete the physicians orders for patient care.
- Case manages client care services for private and MLTC cases.
- Observes and ensures aides are clinically proficient and adequately trained no less often than every three months for private pay patients and every six months for MLTC patients. Supervises field activities of Home Health Aides (HCSS).
- Provides hands-on nursing care during Assessment/reassessment visits.
- Serves as the on-call nurse during assigned weeks, including weekends.
- Performs duties as assigned by the Director of Clinical Services or Executive Director to support program success.
- Participates in case conferences as indicated and collaborates with other team members.
- Maintains patient record in accordance with agency policy and professional standards.
- May take call during non-business hours.
- Carries out all other related duties as assigned.
- Participates in Performance Improvement activities as required.
- Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.
QUALIFICATIONS
- Current licensure to practice as a Registered Professional Nurse in New York
- Minimum of three (3) full-time years clinical nursing
OR
- Current supervised nursing experience providing skilled nursing care in a hospital, certified or long term home health agency or skilled nursing
- Education: Bachelor’s Degree in Nursing (BSN) from an approved school (preferred).
- Maintain clinical competence through formal education, attendance at seminars, workshops, etc., professional involvement and readings.
