OCCUPATIONAL THERAPIST

CenterWell Home Health is seeking an Occupational Therapist to deliver high-quality, in-home therapy services within our Healthcare & Medical Services department. In this role, you will conduct comprehensive patient evaluations, create and implement personalized treatment plans, and focus on improving functional independence, safety, and quality of life. You will collaborate closely with an interdisciplinary care team, provide patient and caregiver education, and maintain accurate electronic documentation.
