Physical Therapist PT Home Health | Danbury Territory

Elara Caring is seeking a Physical Therapist to serve patients throughout the Danbury, Connecticut territory. In this field-based role, you will help patients regain strength, mobility, and independence in the place they feel most comfortable: home.

You will have the independence to manage your patient visits while receiving support from an experienced clinical team.

What You’ll Do

  • Evaluate patients’ mobility, strength, safety, and functional needs
  • Develop and implement individualized treatment plans
  • Help patients improve mobility and safely perform daily activities
  • Educate patients and caregivers on exercises, equipment, and safety techniques
  • Monitor patient progress and adjust treatment plans as needed
  • Communicate changes in condition to physicians and members of the care team
  • Complete accurate and timely clinical documentation
  • Collaborate with nurses, physicians, therapy professionals, and other team members
  • Support patients in reaching their highest possible level of independence

What You’ll Bring

  • Graduate of an American Physical Therapy Association-approved physical therapy program
  • Active and unrestricted Connecticut Physical Therapist license
  • At least one year of experience in a healthcare setting
  • Strong communication, organization, and patient-care skills
  • Ability to work independently in a home-based environment
  • Willingness to travel throughout the assigned Danbury territory
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Home health experience is valuable, but candidates with transferable clinical experience are encouraged to apply.
