Hospitalist Care Coordination Registered Nurse

Intermountain Health is seeking a Registered Nurse Hospitalist Coordinator to support patient-centered, hospital-based care in a collaborative, mission-driven environment. In this role, you will coordinate and deliver acute nursing care for hospitalist patients, guide care transitions, and act as a key liaison among physicians, nurses, patients, and families. Responsibilities include comprehensive assessments, medication administration, discharge planning, and clear documentation in the EHR. You’ll participate in quality improvement, mentor staff, and help drive safe, efficient, and compassionate care aligned with Intermountain’s values.
