CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home care to seniors and adults with chronic conditions. In this role, you will assess functional abilities, develop personalized treatment plans, and provide evidence-based interventions that improve safety, independence, and quality of life at home. You’ll collaborate with an interdisciplinary team, educate patients and caregivers, and document care using modern EMR tools.
