Job Summary:
We are seeking a licensed and experienced LPN to work our evening shift (3:00 PM - 11:00 PM) in our state-of-the-art facility. The ideal candidate will possess excellent clinical skills, a strong work ethic, and a passion for delivering exceptional patient care. Responsibilities:
Provide direct patient care to residents, including medication administration, wound care, and assistance with daily living activities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Participate in quality improvement initiatives to ensure high-quality patient care and safety
Maintain accurate and up-to-date medical records and reports
Collaborate with other healthcare professionals to achieve optimal patient outcomes
Provide emotional support and education to patients and their families
Requirements:
Current LPN license in the state of Massachusetts
Associate degree in Nursing or related field
Ability to work effectively as part of a team
Excellent communication and interpersonal skills
Strong attention to detail and ability to maintain accurate records
