Community Care Physicians seeks a Family Medicine Nurse Practitioner/Physician Assistant to provide comprehensive primary care in our physician-owned, multispecialty group serving New York’s Capital Region. In this outpatient role, you’ll manage acute and chronic conditions, perform exams, order and interpret diagnostics, and deliver preventive and wellness care for patients of all ages.
