Admissions Registered Nurse

Glens Falls Center is seeking an Admissions Registered Nurse to coordinate and oversee the clinical admission process for new residents in our skilled nursing and rehab facility. The RN will complete comprehensive nursing assessments, review medical histories, verify orders, and ensure accurate documentation for a smooth transition of care. This role collaborates closely with physicians, families, hospital discharge planners, and the interdisciplinary team to develop individualized care plans. The Admissions RN educates residents and families on services, supports regulatory compliance, and promotes our resident-focused, compassionate care culture.
