LICENSED PRACTICAL NURSE

CarePartners is seeking a Licensed Practical Nurse to provide compassionate, in-home nursing care to patients across Connecticut. In this field-based role, you’ll deliver skilled services such as medication administration, wound care, vital sign monitoring, and patient education while supporting individualized care plans developed by RNs and therapists. You’ll collaborate closely with an interdisciplinary team, maintain thorough documentation, and promote safety and independence in the home.
