Minimum Requirements Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA Active State License is Required to Start the Assignment BLS Certification May Be Required from AHA or ARC Physical Therapist certification is required to start. CPR (Basic) is required to start. Hep B is required to start. TB is required to start. Flu is required to start. Information Security Compliance Form is required to start.
