Physical Therapist - PRN - Skilled Nursing Facility

We are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents' lives in a rewarding setting.
Position Summary:
As a PRN Physical Therapist, you will evaluate and treat residents to improve their mobility, strength, and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality, individualized care.
Key Responsibilities:
* Perform comprehensive evaluations of residents' physical abilities and needs.
* Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function.
* Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices.
* Monitor residents' progress and modify therapy plans as needed to achieve goals.
* Collaborate with an interdisciplinary team to ensure holistic care.
* Educate residents and their families on therapy goals, exercises, and strategies for continued improvement.
* Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory and facility standards.
* Participate in quality improvement initiatives and contribute to the rehabilitation department's success.
Qualifications:
* Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
* Current and valid Physical Therapist license in the state of Colorado.
* Experience in skilled nursing or rehabilitation settings is preferred.
* Strong communication and teamwork skills with a resident-centered approach.
* Ability to work independently and adapt to a flexible PRN schedule.
