Position Summary
The Transitional Aged Youth Health & Wellness Case Manageroutreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains such as housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and much more. The Case Manager serves as the client’s main advocate and care coordinator within GLIDE’s continuum of care, community partners and resources, and other external systems of care. This position works closely and collaboratively with other TAY HWC team members to enable warm hand-offs and linkages amongst specialists, case managers, navigators, and ambassadors. Upon exit from GLIDE’s care, the Case Manager develops a care transition plan to ensure the clients are linked to additional support services and knows how to reconnect with GLIDE for future support. In addition to 1:1 case management support, this position will also facilitate and/or coordinate weekly activities or psychoeducation groups that will support, promote, and enhance clients’ confidence, wellness, community connection, and growth mindset.
Work schedules may include the need to work on one day, during the weekend. Working hours may also vary, and occur during non-traditional hours.

GLIDE’s buildings are located in the Tenderloin neighborhood. GLIDE maintains an "open door" policy to the community and its clients, who frequently enter and leave the building.
This role is covered by the collective bargaining agreement.
