Kaiser Permanente is seeking an LMSW Case Manager to support inpatient members in Arlington, VA. In this role, you’ll complete psychosocial assessments, coordinate discharge planning, and connect patients with community resources. You will collaborate closely with physicians, nurses, and behavioral health professionals to develop and monitor care plans, reduce readmissions, and ensure safe transitions of care. You’ll document in EMR systems, participate in case reviews, and provide crisis intervention while promoting patient advocacy and culturally competent, evidence-based care.
