Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• AR Physical Therapist license required to start.
• CPR (Basic) required to start.
• HEP B required to start.
• TB required to start.
• Flu required to start.
• Information Security Compliance Form required to start.
• Orientation - Review Annually required to start.
• Signed Job Description required to start.
• Completed Orientation Documents required to start.

• Shift Hours: 8 hours.
