Home Health Physical Therapist

CenterWell Home Health seeks a compassionate Home Health Physical Therapist to deliver high-quality, in-home rehabilitation services. In this role, you will assess functional mobility, develop individualized treatment plans, and provide evidence-based interventions to help adult and geriatric patients regain independence and improve quality of life. Responsibilities include therapeutic exercise, gait and balance training, home safety assessments, and patient/caregiver education. You will collaborate closely with an interdisciplinary team, document care accurately in the EMR, and uphold rigorous standards for safety and clinical excellence.
