Responsibilities
* Provide comprehensive primary care to patients across the lifespan
* Conduct physicals, wellness visits, acute visits and follow-up care
* Evaluate, diagnose and manage acute and chronic conditions
* Provide preventive health counseling and patient education
* Coordinate care with specialists and other healthcare team members
* Utilize telemedicine when appropriate
* Maintain accurate and timely electronic medical records
* Current New York State Nurse Practitioner or Physician Assistant license
* Graduate of an accredited NP or PA program
* Family medicine or primary care experience preferred
* Strong clinical, communication and interpersonal skills
* Commitment to patient-centered, compassionate care
