Licensed Practical Nurse York & Wells Area

What You'll Do

  • Deliver skilled nursing care under the direction of a Registered Nurse and physician

  • Follow and implement individualized Plans of Care

  • Monitor patient conditions and communicate changes to the RN Case Manager and care team

  • Educate patients and caregivers on disease management, medications, and self-care techniques

  • Document assessments, interventions, and patient progress accurately and timely

  • Promote patient safety and implement strategies to reduce avoidable hospitalizations and emergency room visits

  • Collaborate with physicians, nurses, and interdisciplinary team members to ensure quality patient outcomes

  • Support patients and families with compassionate, respectful, and patient-centered care

  • Identify opportunities for improvement and communicate recommendations to the RN Case Manager

What You'll Bring

  • Graduate of an accredited Associate Degree Nursing program

  • Current state LVN/LPN license

  • Minimum of one (1) year of clinical nursing experience required

  • Home Health or Hospice experience preferred not required

  • Strong communication, critical thinking, and patient care skills

  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

  • Willingness to travel within the assigned service area
