Physical Therapist – Home Health

Boston Proper | $10,000 Sign-On Bonus

Imagine spending your day helping patients navigate the moments that matter most—walking safely through their homes, rebuilding strength after hospitalization and regaining the confidence to live more independently.

Elara Caring is seeking a Physical Therapist to provide personalized, one-on-one care to patients throughout Boston Proper. This is an opportunity to practice with greater autonomy, build meaningful patient relationships and see how your expertise translates into real-life progress.

The Opportunity at a Glance

Salary: $100,000–$115,000 annually
Sign-On Bonus: $10,000
Schedule: Monday–Friday (no weekends or holidays)
Territory: Boston Proper
Weekends, Holidays and On-Call: None required

Final compensation is negotiable within the posted range and based on experience, education, qualifications and internal equity.

The Difference You’ll Make

  • Complete comprehensive evaluations in each patient’s home

  • Develop treatment plans centered around meaningful, functional goals

  • Help patients improve strength, balance, mobility and overall safety

  • Support recovery following surgery, illness, injury or hospitalization

  • Teach patients and caregivers how to use exercises and equipment safely

  • Identify fall risks and recommend strategies for a safer home environment

  • Monitor progress and adjust therapy plans as patient needs change

  • Collaborate with nurses, physicians, therapists and clinical leadership

  • Complete accurate and timely clinical documentation

Your Qualifications

  • Graduate of an American Physical Therapy Association-approved program

  • Current, unrestricted Massachusetts Physical Therapist license

  • Valid driver’s license, reliable transportation and current auto insurance

  • Ability and willingness to travel throughout Boston Proper

  • Strong clinical judgment, communication and time-management skills

  • Ability to work independently while remaining connected to a collaborative team

  • Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
