Portland, OR. This position involves providing outpatient rehabilitation services to patients in a home health setting. The role requires delivering patient-centered care through various therapeutic techniques, with a focus on improving mobility and functional outcomes.

Job Details

Location: Portland, OR

Shift: 8-hour days

Schedule: 40 hours per week, start date 09/21/2026

Qualifications: Valid state license (compact license accepted), BCLS certification. Minimum of 0 years of experience required.

Preferred Qualifications: Experience in outpatient clinic, rehabilitation hospital, SNF, or home health environments; proficiency with therapeutic equipment and modalities listed in the skills section.

Patient Ratios: Up to 6 patients per day with a 1:1 ratio, efficiency in patient visits is expected.

Additional information: Candidates must have their own transportation, as travel within an approximate 60-mile radius is required. Flexibility for occasional drive time is considered.

Orientation: Details to be provided pre-start, hours to be completed prior to beginning patient care.

Other Requirements: Ability to work as part of an interdisciplinary team, adherence to safety protocols, and completion of required modules pre-start and during orientation.
