Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current Physical Therapy licensure in the state of Kentucky
• Current CPR certification
• Current driver's license and vehicle insurance, and access to a dependable vehicle

Assignment Details

• Monday through Friday schedule.
• Shift hours: 0800-1630.
• 40 hours per week.
• No on-call requirement.
• Coverage area includes Hardin County, Meade County, and Breckinridge County.
• May float to other nearby home health agencies as needed.
• Mileage is reimbursed from the first patient's home to the last patient's home.
