Registered Nurse Case Manager

CenterWell Home Health is seeking a Registered Nurse Case Manager to provide in-home, patient-centered care for seniors and adults with chronic conditions. In this role, you will coordinate and manage individualized care plans, perform comprehensive assessments, deliver skilled nursing services, and educate patients and caregivers on disease management and medications. You will collaborate with an interdisciplinary team, use electronic documentation tools, and ensure compliance with clinical and regulatory standards while supporting patients’ independence at home.
