Float Physical Therapist

VNS Health is seeking a Float Physical Therapist to deliver high-quality, patient-centered home and community-based rehabilitation services across New York City. In this flexible float role, you will travel between patient homes and community settings, providing comprehensive PT evaluations, evidence-based interventions, and discharge planning for diverse adult and geriatric populations. You will collaborate closely with an interdisciplinary care team, educate patients and caregivers on mobility, safety, and exercise programs, and document care in accordance with regulatory and agency standards.
