Inpatient Care Management Registered Nurse II

Mercy seeks an RN Care Manager II for Mercy Hospital St. Louis to coordinate inpatient care and ensure safe, efficient transitions. In this role, you will conduct comprehensive assessments, identify risks and discharge barriers, and collaborate closely with physicians, nurses, and ancillary teams to develop individualized care and discharge plans. You’ll coordinate post-acute services, community resources, and patient education while monitoring utilization, quality outcomes, and readmission risks.
