Now Hiring: Wellness Coordinator
Job Summary:
The Wellness Coordinator (LPN) develops, updates and coordinates the patients' plan of care to achieve patient goals and to optimize outcomes and transitions across the care continuum. They provide skilled nursing services and administer medications in the home environment according to the facility's plan of care for the patients. They foster a learning environment for patients/families, nursing, and other members of the healthcare team, including students, and facilitate formal and informal learning for patients/families, nursing, other members of the healthcare team, and the community. An RN Coordinator aligns patient safety, self-safety, and environmental safety as equally important for the best outcomes.

Qualifications of a Wellness Coordinator:
Relevant certifications in gerontology or wellness programming are a plus.
Previous experience in a similar role, preferably in a nursing home or long-term care setting.
Strong interpersonal skills and empathy when working with elderly residents.
Ability to create and adapt wellness programs to suit various physical abilities and cognitive levels.
Excellent organizational and communication skills.
