The Opportunity


Florence Health is seeking an experienced Registered Nurse to join our Practice Partner team and support a portfolio of independent primary care practices participating in our value-based care programs. Reporting to the Head of Network Operations, you will serve as the bridge between our provider partners and our internal product, analytics, and operations teams.


This is a role for a nurse who has already moved beyond direct patient care into population health, care management, quality, or value-based care operations. You will act as a trusted clinical resource to practices across our MSSP and ACO LEAD populations—evaluating care processes, closing gaps, and driving measurable improvement in cost, quality, and patient outcomes across populations rather than at the bedside.


Territory


This role supports a portfolio of 12–15 primary care practices across the United States. Candidates may live anywhere in the U.S.


What You’ll Do


  • Own a portfolio of ~12–15 independent primary care practices across our MSSP and LEAD populations


  • Review clinical and financial performance with providers and recommend targeted interventions


  • Drive provider adoption of Nightingale—pre-visit reviews, between-visit worklists, and risk-stratified alerting


  • Serve as a trusted clinical resource to practices for performance improvement


  • Prepare and present reports across cost, quality, and patient outcomes, partnering cross-functionally on scalable solutions


What We’re Looking For


  • Registered Nurse (RN) with 5+ years of clinical experience; acute care experience preferred


  • Prior experience in a non-bedside role—care or case management, population health, utilization management, quality improvement, clinical informatics, or value-based care / ACO operations (required); this role is not a first move out of direct patient care


  • Active, unencumbered RN license; multistate (NLC) license preferred (additional state licensure may be required depending on assigned territory)



  • Strong clinical judgment and a working understanding of primary care workflows and patient management


  • Excellent analytical and organizational skills; comfortable interpreting performance metrics to inform decisions


  • Strong communication and relationship-building skills across multidisciplinary teams


  • Experience with value-based care, ACOs, or Medicare programs preferred


  • Comfort operating in a fast-paced, entrepreneurial environment


Travel


This role is remote with quarterly travel across the territory to meet with provider partners and internal teams.
