License Clinical Social Worker

HarmonyCares is seeking a Licensed Clinical Social Worker to provide compassionate, home-based support to medically complex and home-limited patients. In this role, you will complete psychosocial assessments, deliver counseling and crisis intervention, and partner with physicians, nurses, and other team members to coordinate comprehensive in-home care. You will help patients and families navigate community resources, advance care planning, and palliative care needs. Working in a patient-first, mission-driven environment, you will make a direct impact on outcomes while contributing to a respectful, collaborative, interdisciplinary team.
