Physical Therapist - Home Health

Adventist Health is seeking a compassionate Physical Therapist to provide high-quality home health services. In this role, you will perform comprehensive evaluations, design and deliver individualized treatment plans, and help patients regain mobility, strength, and independence in their own homes. You’ll collaborate closely with an interdisciplinary team, document care in the EHR, and educate patients and caregivers on safety and home exercise programs.
