Physician Assistant

Signify Health is seeking a Physician Assistant for In-home Visits to deliver high-quality care to members in their homes. In this role, you will perform comprehensive health assessments, review medical histories and medications, and identify gaps in care for largely vulnerable and complex populations. You will develop care plans, document in an EHR, and collaborate with physicians, care coordinators, and health plans to support value-based care objectives. Working independently in the field, you’ll use modern technology and data-driven insights to improve outcomes while enjoying schedule flexibility and work–life balance in a mission-driven, collaborative culture.
