Physical Therapist I – Home Health

10,000 Sign on Bonus

Schedule: Mon-Friday 8 am-5 pm

Territories open: Multiple positions open in : Boone, Hendricks, Putnam, Hamilton, Marion, Johnson and Shelby Counties

Summary
Provides skilled physical therapy services in patients' homes under physician orders. Evaluates, develops, and implements individualized treatment plans to restore function, reduce pain, improve mobility, and maximize independence. Ensures timely documentation, patient safety, and compliance with Medicare, state, and agency regulations.

Key Responsibilities

  • Perform patient evaluations, OASIS assessments, and develop individualized plans of care.
  • Provide skilled physical therapy treatments and adjust care plans based on patient progress.
  • Educate patients and caregivers on disease management, exercises, mobility, fall prevention, and use of assistive devices.
  • Collaborate with physicians, interdisciplinary team members, PTAs, and home health staff to ensure continuity of care.
  • Supervise PTAs and complete required supervisory visits.
  • Document evaluations, treatments, patient education, and care plans accurately within required timeframes.
  • Participate in case conferences, on-call rotation, staff meetings, and ongoing education.
  • Travel to patient homes for scheduled visits.

Qualifications

  • Current Physical Therapist license in the state of employment.
  • Minimum one year of acute care experience required; home health experience preferred.
  • Knowledge of OASIS, Medicare regulations, and physical therapy best practices.
  • Strong communication, clinical decision-making, and organizational skills.
  • Valid driver's license, reliable transportation, and current auto insurance.

Physical Requirements
Frequent standing, walking, bending, lifting up to 50 lbs. (with or without assistance), driving, and computer documentation.
