Licensed Social Worker

Kaiser Permanente seeks a Licensed Social Worker for our ambulatory clinic in Kailua Kona. In this role, you will provide psychosocial assessments, brief counseling, care coordination, and crisis intervention for patients in an outpatient setting. You will collaborate closely with physicians, nurses, and care teams to develop care plans, connect patients with community resources, and support adherence to treatment. You’ll document in the electronic health record, contribute to care improvement initiatives, and help integrate behavioral health into a technology-enabled, patient-centered environment.
