Signify Health is seeking a Collaborating Physician to provide remote clinical oversight for advanced practice clinicians performing in-home evaluations for vulnerable patient populations. In this part-time role, you will review and co-sign charts, guide complex clinical decisions, and ensure adherence to evidence-based and value-based care standards.
