In this role, you’ll provide high-acuity, patient-centered care for adults transitioning from critical care, closely monitoring complex conditions, titrating medications, and collaborating with an interdisciplinary team. You will perform comprehensive assessments, implement evidence-based care plans, respond rapidly to changes in status, and educate patients and families.
