This Home Health Physical Therapist role is based in Salisbury, Maryland, a charming city on the Eastern Shore that combines small-town warmth with access to outstanding outdoor recreation and coastal amenities. As a Home Health Physical Therapist, you will provide one-on-one, evidence-based physical therapy services to adult and geriatric patients in their homes throughout Salisbury and surrounding communities. You will play a key role in helping patients regain independence, improve mobility, reduce fall risk, and safely remain in their preferred home setting. This position is ideal for clinicians who value autonomy, meaningful patient relationships, and the ability to see firsthand how therapy impacts daily life. Typical responsibilities include performing comprehensive physical therapy evaluations in the home, identifying functional limitations, home safety risks, and patient-centered goals. You will develop and implement individualized treatment plans focused on gait training, balance and strengthening programs, transfer and stair training, fall prevention strategies, pain management, and education on energy conservation and safe mobility. You will assess the need for assistive devices and durable medical equipment and provide training to both patients and caregivers on proper use. You will work closely with an interdisciplinary home health team, collaborating with nursing, occupational therapy, speech therapy, and medical social work to coordinate care, update plans of care, and support smooth transitions from hospital or facility to home. Clear, timely electronic documentation in the agency’s EMR system is an essential part of the role, ensuring regulatory compliance and contributing to quality metrics and clinical outcomes. The schedule typically follows daytime hours with flexibility based on patient needs and visit volumes. Caseloads and daily visits are structured to allow for thorough assessments, interventions, patient and caregiver education, and travel time between homes. Patient acuity and visit expectations are designed to support safe, high-quality care rather than rushed encounters. You will encounter a variety of clinical presentations, including post-acute and post-surgical patients, individuals with chronic conditions, neurologic diagnoses, and those recovering from injury or deconditioning.
