Physical Therapist - Travel Contract

Minimum Requirements
* Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
* Active State License is Required to Start the Assignment
* BLS Certification May Be Required from AHA or ARC
* CPR (Basic) is required to start.
* Hepatitis B documentation is required to start.
* TB documentation is required to start.
* Flu documentation is required to start.
* Information Security Compliance Form is required to start.
* Orientation - Review Annually is required to start.
* Signed Job Description is required to start.
* Completed Orientation Documents is required to start.
Assignment Details
* Shift schedule: 5x8.
* Hours: 12:00 AM - 12:00 AM.
