CenterWell Home Health is seeking a Weekend Home Health Physical Therapist to deliver compassionate, one-on-one care to seniors and adults with chronic conditions in their homes. You will perform comprehensive evaluations, create evidence-based treatment plans, and provide hands-on therapy focused on mobility, safety, and independence. Collaborate with an interdisciplinary team, educate patients and caregivers, and document care in our EMR.
