RN Care Manager

Martin Luther King, Jr. Community Hospital seeks an RN Care Manager for our Emergency Room to coordinate care for a diverse, high-acuity patient population in South Los Angeles. In this on-site role, you will perform comprehensive nursing assessments, develop and manage individualized care plans, and collaborate closely with physicians, social workers, and the interdisciplinary team to ensure safe transitions of care. You will support discharge planning, connect patients with community resources, and provide culturally responsive education to patients and families. Ideal candidates bring emergency or acute care RN experience, strong care coordination skills, and a passion for health equity in underserved communities.
