SOCIAL WORKER

Sanford Health is seeking a Senior Social Worker to provide advanced clinical support within our Healthcare & Medical Services team. In this role, you will conduct psychosocial assessments, develop care and discharge plans, and coordinate community resources for patients and families. You will collaborate closely with physicians, nurses, and interdisciplinary teams to ensure safe transitions of care and optimal outcomes. As a senior member, you will mentor junior staff, support quality improvement initiatives, and uphold Sanford Health’s culture of compassionate, inclusive, and innovative care.
