Senior Community Care is seeking a compassionate Nurse Practitioner to provide primary and preventive care to seniors enrolled in our PACE program. In this interdisciplinary role, you will perform assessments, diagnose and manage acute and chronic conditions, prescribe medications, and develop individualized care plans. You’ll collaborate closely with physicians, nurses, social workers, therapists, and caregivers to support safe, independent living.
