Inpatient Hospital Medical Social Worker

Kaiser Permanente seeks an Inpatient Social Worker Case Manager within our Healthcare & Medical Services department. In this role, you will provide psychosocial assessment, discharge planning, and care coordination for hospitalized patients and families. You will collaborate with physicians, nurses, and interdisciplinary teams to ensure safe transitions of care, connect patients to community resources, and address social determinants of health. You will also support crisis intervention, facilitate family meetings, advocate for patient rights, and maintain accurate documentation in our electronic health record to ensure high‑quality, patient‑centered care.
