The Role

As an

Advanced Practice Provider (NP or PA) , you will play a critical role in delivering comprehensive survivorship care for patients navigating life after cancer treatment.
This is a

telehealth-first position —all visits are conducted virtually—allowing you to combine flexibility with meaningful, high-touch patient care.
What You’ll Do

Conduct

virtual survivorship visits

and develop individualized, patient-centered care plans
Manage

long-term and late effects of cancer treatment , including chronic conditions
Coordinate

evidence-based surveillance and recurrence monitoring
Provide

education, counseling, and psychosocial support

via telehealth
Collaborate with a

multidisciplinary care team

(oncology, behavioral health, care navigation)
Help design and refine

clinical workflows, SOPs, and care protocols
Contribute to

quality improvement, patient experience, and program scalability
Who We’re Looking For

Nurse Practitioner (NP) or Physician Assistant (PA)

with active MO license (or ability to obtain a MO state licenses)
Strong clinical judgment and comfort delivering care in a

virtual environment
Excellent communication skills with a

patient-first, empathetic approach
Ability to thrive in a

fast-paced, evolving startup environment
Highly organized with strong attention to detail
