What You’ll Do
- Deliver skilled nursing care to patients in their homes throughout assigned Williamsport communities
- Develop and manage plans of care in collaboration with physicians and interdisciplinary partners
- Provide patient and family education that promotes independence and recovery
- Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop
- Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards

Qualifications
