Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapist license is required to start the assignment.
  • BLS Certification from AHA or ARC may be required.
  • Experience working in Skilled Nursing Facility settings preferred.
  • Experience with mobility training, ADL restoration, and fall prevention programs preferred.
  • Documentation experience with common rehab software and electronic medical records preferred.
