Hospice Admission/Liaison Registered Nurse

Tufts Medical Center seeks a compassionate Hospice Admission/Liaison Registered Nurse to guide patients and families through the hospice admission process and coordinate seamless transitions of care. In this field-based role, you will conduct comprehensive assessments, explain hospice services, obtain consents, and develop initial care plans aligned with patients’ goals. You’ll act as a key liaison between referring providers, inpatient teams, and the hospice care team, ensuring timely communication and high-quality, patient-centered care. You’ll document in the EMR, support symptom and pain management at admission, and educate families on end-of-life care, reflecting Tufts’ commitment to innovation, collaboration, and community health.
