The Avenue Care and Rehabilitation is a post-hospital rehabilitation and skilled nursing care facility offering a distinctive experience not commonly found in traditional care settings. We are proud to provide some of the area’s highest levels of rehabilitation, nursing, and medical support for individuals transitioning from hospital to home. Our team is dedicated to delivering compassionate physical, medical, emotional, and spiritual care in a respectful and uplifting setting.

We’re looking for compassionate and skilled Floor Nurses to provide direct care and support to our residents. If you’re a licensed LPN or RN who thrives in a team environment and values quality care, we’d love to meet you!

Job Duties:

  • Deliver nursing care based on individualized care plans

  • Administer medications and treatments as prescribed

  • Monitor and document resident conditions and responses

  • Assist with admissions, discharges, and transfers

  • Supervise nursing assistants and collaborate with care teams

  • Participate in care planning and assessments

  • Respond to emergencies and ensure regulatory compliance

Qualifications:

  • Current LPN or RN license in the state of employment

  • CPR certification (AHA, Red Cross, or equivalent)

  • One year of long-term care experience preferred

  • Strong clinical, communication, and organizational skills

  • Ability to lift up to 50 lbs and work flexible hours
