Licensed Practical Nurse (LPN)

Location: Ditmas Park, Brooklyn, NY
Compensation: $30–$34 per hour

Schedule: Full-Time | Day, Evening, and Night Shifts Available

Ditmas Park Nursing & Rehab is seeking compassionate and dependable Licensed Practical Nurses to join our nursing team.

The LPN will provide high-quality care to residents while working closely with RNs, physicians, CNAs, and other members of the interdisciplinary team.

Responsibilities:

  • Administer medications and treatments as ordered

  • Monitor residents and report changes in their conditions

  • Maintain accurate and timely clinical documentation

  • Assist with admissions, transfers, and discharges

  • Communicate with physicians, residents, and families

  • Supervise and support CNAs during the assigned shift

  • Follow all infection-control and resident-safety protocols

Requirements:

  • Active New York State LPN license

  • Strong clinical and communication skills

  • Dependable, compassionate, and team-oriented

  • Nursing-home or long-term-care experience preferred
